REQUEST FOR PROPOSALS
BAYONNE BOARD OF EDUCATION

This Public Notice amends a previous Public Notice for Food Service Management Company Services and Engineering Services that was advertised on April 13, 2013.  The Bayonne Board of Education is no longer requesting proposals for Engineering Services at this time, and will not have an RFP opening for these services.  The Bayonne Board of Education is also postponing the opening for Food Service Management Company Services, to Wednesday, May 15, 2013.

The Bayonne Board of Education, 669 Avenue A, Bayonne, NJ 07002 will receive sealed proposals for the following services as listed below.  This request for proposals shall utilize the competitive contracting process (as determined by N.J.S.A. 18A-4.1k and Local Finance Notice 2010-3) and a “fair and open” contracting process (as defined in N.J.S.A. 19:44-20.7):

These services, as well as the selection criteria below, are more fully explained in the Request for Proposals, which is available on the Bayonne Board of Education’s website, www.bboed.org.

SELECTION CRITERIA OF PROVIDERS:
Regarding the method of award of submissions, the right is reserved by the Bayonne Board of Education to award submissions on a “service by service” basis, “per project basis” in part or in whole as determined by the Owner. 

The selection criteria shall include evaluation of the content of the proposals.  Each interested provider shall submit a proposal with the following information setting forth its qualifications that will be evaluated among the selection criteria listed below.
1. Full business name, address, and other important contact information
2. Evidence of Applicant Capability, in general, and as it specifically relates to the areas described in the Request for Proposals.
3. Specific approach to areas in narrative responses to Request for Proposal requirements.
4. Technical Process, Materials and Equipment: Description of any technical process, materials (including specialized software) and equipment used in performing the services.
5. Cost Details: Cost details itemized by each specific area outlined in the Request for Proposals, which may include hourly or unit rates where appropriate.  A grand total proposed cost shall be provided.  The “Response & Projected Operating Statement – Form 23” shall be used in providing cost details on the Proposal.
6. Mandatory items as required by law.

SEALED RESPONSES SHOULD INCLUDE:
1. The “Proposal”, signed and dated by the provider, which shall include
a. The body of the Proposal, which shall include:
i.  A Proposal Price Quotation Sheet showing a grand total proposed cost, and cost details itemized by each specific area outlined in the Request for Proposals.
ii.  Addressing Selection Criteria as set forth above.
iii. Clearly referencing any additional sheets or attachments.
            b. A signed and notarized “Non-Collusion Affidavit”.
            c. A signed “Stockholder Disclosure Certification”.
            d. A signed acknowledgment of the American with Disabilities Act of 1990 (Appendix A)
2. The following items may be submitted with your proposal at your option, but must be provided prior to the execution of a contract sought under this RFP.
a. An Affirmative Action Statement (Exhibit A)
b. A copy of the vendor’s current “NJ Business Registration Certificate.”  Information on this certificate can be obtained at “www.state.nj.us/treasury/revenue/busregcert.shtml.”
c. IRS Form W-9
3. Such other documents and materials as may be appropriate to show the qualifications and experience of the provider or to meet the requirements of this submission.

Items 1b, 1c, 1d, 2a and 2c above are included as attachments to the Request for Proposal on the Board’s website “www.bboed.org”.

SUBMISSION DEADLINE:  Sealed proposals must be received in the Business Office, Bayonne Board of Education, 669 Avenue A, Bayonne, NJ 07002 on or before 10:00AM on Wednesday, May 15, 2013 (“due date” set forth in this notice, also referred to as the “submission deadline”).  Proposals will be publicly opened and read later that day at 2:00PM, at the Administrative Conference Room, Bayonne Board of Education, 669 Avenue A, Bayonne, NJ 07002. The Bayonne Board of Education Assistant Superintendent for Business and/or his designated representative will receive submissions up to the submission deadline noted above. The Bayonne Board of Education reserves the right to extend the submission deadline at any time prior to opening of the sealed submissions, to reject all submissions without the need for cause or prior notice, to reject particular submissions due to defects in mandatory items, to waive non-mandatory items and to accept any submissions that in their judgment will be in the best interest of the Bayonne Board of Education.

If awarded a contract, your company/firm shall be required to comply with the requirements of N.J.S.A. 10.5-31 et seq. and N.J.A.C. 17:27.
